Students must submit completed physical examination forms with proof of immunities and submit payment to the Program for additional supplies as outlined in the acceptance letter sent certified mail. Note: Applications are accepted February 1- August 1. Submission of the Dental Assisting Program application with transcripts is required prior to enrollment.
